Instructions

This is the official PHQ-15 questionnaire. It asks one thing: how much you have been bothered by physical complaints over the past 4 weeks. For each of the 15 items, choose the answer that best describes that period.

If an item does not apply to you β€” the question about periods, for example β€” choose the first answer. It counts as 0 points, exactly what a skipped item is worth.

Important: this questionnaire measures how much symptoms bother you β€” it does not establish where they come from. A high score does not mean your symptoms are "all in your head", and a low one does not rule out illness. Symptoms such as chest pain, fainting or shortness of breath always need a medical assessment, whatever this questionnaire says.

The items and response scale come from the PHQ-15, developed by Drs. Robert L. Spitzer, Janet B.W. Williams, Kurt Kroenke and colleagues with an educational grant from Pfizer Inc. No permission is required to reproduce, translate, display or distribute it (phqscreeners.com). The Polish, English, German and Spanish versions are the licensor's official translations; in Japanese and Turkish the sleep and tiredness items are official and the rest are our own translations.

The cutpoints of 5, 10 and 15 are this questionnaire's published thresholds, not ours. Even so, the test is for information only and does not replace a diagnosis. It measures how severe your complaints are, not what causes them β€” physical symptoms need a medical assessment whatever your score.
