Have you ever experienced an unusually frightening or upsetting event, such as a serious accident or fire, physical or sexual assault or abuse, an earthquake or flood, war, witnessing someone being killed or seriously injured, or a loved one dying through homicide or suicide?

You can skip or stop if distressed. Without such an event, the five-item PC-PTSD-5 screen is unnecessary; a score of 0 is not mental-health clearance. If unsure, you can choose a general self-check or discuss it with a professional.

Answers stay in your browser until submission. With explicit consent, see your result immediately without an account or email. Saved results link to your browser session, without answers, IP addresses or full browser details. Private access lasts 30 days; daily cleanup deletes expired results from the active database within one day. Analytics needs separate consent; answers, results, email and topics are excluded.

To withdraw storage consent, you can delete the saved result from its result page in the browser where you completed the questionnaire. If that browser session is lost or you open an email link on another device, contact the privacy team for deletion.

PC-PTSD-5 (Primary Care PTSD Screen for DSM-5) · US VA National Center for PTSD

For PC-PTSD-5, we use 3 as a sensitive positive-screen threshold. VA reports that 4 performed well overall and for men; a lower threshold may reduce false negatives in some samples, including women. No clinical cutoff applies universally across settings and languages. A positive or negative score neither establishes nor excludes PTSD.

Published cutoffs do not validate edited questions, translations or added score bands. Use them as informational context. If you are concerned, seek a professional assessment regardless of your score.

Can this questionnaire diagnose a condition?

No. It is a snapshot based on how you report feeling at this time. It cannot confirm or rule out a condition.

Do I need an email, and how long is my result kept?

No. You can see your result without an account or email. Email is optional, for one requested report. The waitlist has a separate checkbox, unchecked by default. Private access lasts 30 days. Expired results are deleted from the active database within one day of expiry. Backups have separate retention periods described in the privacy policy.
